If your research meets the definition for "exempt", then you need to submit

1. An IRB Application (Login to MyPark required)
2. A letter of informed consent
3. Copies of all questionnaires, interview protocols, etc.

If your research meets the definition for "expedited review", then you need to submit

1. An IRB Application (Login to MyPark required)
2. A letter of informed consent
3. Copies of all questionnaires, interview protocols, etc.

No IRB applications will be reviewed during the last month of the Fall or Spring Semesters of the Park University Home Campus Day Program.    DO NOT BEGIN TO COLLECT DATA UNTIL AFTER YOU HAVE RECEIVED WRITTEN APPROVAL FROM THE IRB.

If your research meets the definition for Full Board review, then you need to submit:

1. An IRB Application (Login to MyPark required)
2. A letter of informed consent
3. Copies of all questionnaires, interview protocols, etc.

DO NOT BEGIN TO COLLECT DATA UNTIL AFTER YOU HAVE RECEIVED WRITTEN APPROVAL FROM THE IRB.
